The following documents must be on file prior to the confirmation and pick up of any rental reservation.

We offer convenient pick up locations throughout the NY, NJ, CT, and PA areas.  However, most of these locations are warehouses ONLY, and some do not process paperwork.  For this reason, we require that you either FAX or SCAN AND EMAIL your documents to us BEFORE picking up your equipment.  If you bring these documents with you to the warehouse, they will NOT release the equipment to you - again, as stated, they do not concern themselves with paperwork.  They are busy making sure that your equipment is in good working order, and ready for pick up at your convenience.

If you do not see the E-SIGN forms below, or you'd rather print & fax your forms, you may download each form individually here:

  1. RENTAL AGREEMENT
  2. CREDIT CARD AUTHORIZATION (please include a copy of VALID ID & CREDIT CARD)


Unless otherwise instructed or other arrangements have been made with our staff, you must submit both forms along with a copy of VALID I.D. and CREDIT CARD used to pay for the order.

VALID Forms of ID
We cannot accept EXPIRED ID, School ID, Temporary ID, or Foreign/Int'l ID Cards.

        • State ID/Driver's License
        • Current Passport
        • Corporate Business ID (certain restrictions apply) - Business Card, Employment Letter on Company Letterhead, Business ID Card
        • Military ID (active, retired, or reserve)
